Polymorbidity and polypharmacy are common in the elderly patients and combined with the age-related changes in the body are associated with a higher risk of adverse drug reactions (ADRs). Numerous tools, e.g. EURO-FORTA list, have been developed to improve the quality of pharmacotherapy in the elderly. EURO-FORTA list was validated in several clinical studies and is convenient for use in real-life practice. It helps to reduce polypharmacy, to improve a number of parameters of a comprehensive geriatric assessment and to reduce the number of ADRs. In 2023, European experts presented an improved EURO-FORTA list, which contains 267 prescriptions (medicines or pharmacotherapeutic groups) used in treatment and most often prescribed to elderly patients, in accordance with 27 main indications for use.
